Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A subsidy health and safety inspection was initiated on May 6, 2021 and concluded on May 7, 2021. The director was contacted by telephone to initiate the inspection. There were nineteen children present and two staff. A licensing inspection was conducted on the same date as the SHSI supplemental inspection. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection. The information gathered during the inspection determined non-compliance(s) with subsidy regulations, and violations were documented on the violation notice issued to the facility.

Based on review and interview, facility did not ensure 1 of 2 staff complete the department's required update training.

Evidence: The record for staff #2 had documentation of completing the preservice training on 2/27/2019 and the department's health and safety update training had not been done. Staff confirmed the training had not been completed.
Plan of Correction: Staff will complete the training.
