Inspector Notes
A subsidy health and safety (SHSI) inspection was conducted on September 25, 2023 from approximately 3:50 pm to 4:50 pm. There were fourteen children present with two staff supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection. Violations were found and noted on the violation notice.

Evidence: The records for staff #1 (start date 3/6/2023) and staff #2 (start date 9/28/2022) did not have documentation of completing the Virginia Preservice Training.
