Inspector Notes

An unannounced monitoring/modification inspection was conducted on December 1, 2022 from 9:15 AM -11:30 AM. Today?s census included 50 children with 8 staff supervising. Upon arrival the children were playing outside and playing with various play materials in the classroom. The inspector observed 4 classrooms, and restrooms. Outside play area was not observed due to the weather. The center has submitted a modification document requesting an increase in the capacity. Measurements for each classroom were completed previously. During the inspection, the licensing inspector reviewed the following: emergency supplies including first aid kit, battery operated weather radio, emergency plan, emergency drill logs, menu, caregiver?s training hours and daily attendance. A sampling of medication was reviewed. Transportation is not provided. During the inspection five children's records, five staff records and four board member records were reviewed. Based on record review, the center failed to ensure staff obtain and submit the results of a follow-up tuberculous (TB) screening at least every two years. Evidence: 1. Staff records for Staff #1 DOH 9/13/2020 and Staff # 5 DOH 9/9/2008, did not have documentation of a TB Screening obtained every two years from the date of the initial screening or testing.
Plan of Correction: Staff advised to complete TB test ASAP.

Based on review of staff records, the center failed to ensure that four out of five staff completed orientation training prior to working alone with children and no later than seven days of the date of assuming job responsibilities.
