Inspector Notes

A complaint inspection was initiated on 8/23/2023 from approximately 9:15 AM until 10:30 AM regarding a complaint received on 6/26/2023 regarding the terms of the license. Additional attempts were made to initiate this complaint on 7/10/2023 and on 7/25/2023. Interviews with staff were completed, and documentation was reviewed. The preponderance of the evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. See the violation notice for more details.

Based on interview and review of records, the center did not maintain compliance with the terms of the issued license.
Evidence:
1. The center is licensed for ages 2 years 6 months through 6 years 11 months as stated on the license.
2. Attendance sheets verified that children between the ages of 7 years through 12 years were in attendance during the afterschool hours from approximately 3:00 PM until 6:00 PM.
3. Staff verified that children between the ages of 7 years through 12 years were participating in after care when needed. After care is offered to all children that is enrolled in the private school.
Plan of Correction: Have submitted request to inspector to increase in age on license to 12 years. It was noted that RWS previously held a license through the age of 12, and that records indicate it was a clerical error on the last renewal application.
