During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on June 3, 2025. The on-site inspection began at 10:00 am and ended at 12:20 pm. The inspector reviewed compliance in the areas listed above. There were 56 children present and 8 staff. The inspector reviewed 5 children?s records and 5 staff records on-site. This inspection included a tour of the facility, observations, interviews, and document review.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-140-A
(Repeat violation) Physical examinations completed by a physician shall be obtained prior to the first day of attendance or within 30 days after the first day of attendance.

There was no physical examination in the record of child #3, who had been enrolled for three years and 10 months.
Plan of Correction: Will request physical exam page of form from parent by 6/30/2025. Parent sent over all pages of the school entrance health form but the physical exam page did not scan through correctly. Page was cut off.
Standard 8VAC20-780-260-A
The center shall provide to the licensing representative an annual fire inspection report from the appropriate fire official having jurisdiction.

The most recent fire inspection was overdue by 3 months. The last fire inspection report was dated 2/9/2024.
Plan of Correction: Complete fire inspection by 6/30/2025.
Standard 8VAC20-780-40-E
The operational responsibilities of the licensee shall include ensuring that the center?s activities, services, and facilities are maintained in compliance with the center?s own policies and procedures that are required by the standards.

The center?s medication procedures states ?medications not retrieved from the front office within 2 weeks of the earlier expiration of permission or the last day of school will be discarded?. Child #7 had prescription medication at the center that expired three weeks ago.
Plan of Correction: Will return medication 6/3/2025 and request new medication from the parent.
