Inspector Notes

A renewal inspection was initiated on January 11, 2022 and concluded on January 13, 2022. There were six school age children present with one staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of two child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review and interview, facility did not ensure there is always one staff member on duty who has obtained daily health observation training within the last three years.

Evidence: On 1/11/2022 at approximately 4:15 pm, there was not a staff member present that was trained in daily health observation within the past three years. Staff #1 was present and the training was last completed on 6/4/2018.

Based on observation and interview, facility did not ensure when one or more children are present there are two staff present or one staff and an immediately available staff, volunteer or other employee available on the premises with direct means of communication between the two of them.

Evidence: On 1/11/2022 at approximately 4:15 pm, staff #1 was present with six school age age children. Staff stated the second staff member scheduled to work called in sick on this day.
