Inspector Notes

A monitoring inspection was initiated on February 23, 2023 and concluded on February 28, 2023. There were twelve children present with two staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of five child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Based on review, facility did not ensure 2 of 5 child records reviewed have documentation of required information.

Evidence: The following was found during record review:
1. The record for child #2 (start date 12/5/2022) did not have emergency contact addresses and physician information (name and phone number).
2. The record for child #3 (start date 9/7/2022) did not have a second emergency contact, physician information (name and phone number) and allergy action plan for a food allergy noted on the child's physical examination.
