Inspector Notes
An unannounced renewal inspection was conducted on February 5, 2024 from approximately 4:30 pm to 5:15 pm. Two staff were present with five school age children. Children were observed participating in various table activities in the cafeteria. Five child and two staff records were reviewed. Violation was found and is noted on the violation notice.

Based on review and interview, facility did not ensure 3 of 5 injury reports have documentation of required information.
Evidence: The following was found during review of injury reports:
1. The record for child #2 had documentation of injury sustained on 10/2/2023. The parent signed the form on 10/5/2024. Site Director stated the parent was notified on the day of incident but that date was not documented.
2. The record for child #4 had documentation of injury sustained on 9/29/2023 and the time of parent notification was not documented.
3. The record for child #5 had documentation of injury sustained on 9/20/2023 and the time of parent notification was not documented.
4. Site Director confirmed missing information.
