The center shall obtain documentation that each child has received the immunizations required by the State Board of Health before the child can attend the center. Child #2 attended the center for one week before submitting documentation of an immunization for the child?s record.

The medication authorization shall be available to staff during the entire time it is effective. Prescription medication was present at the center for Child #9. A completed medication authorization form for the medication prescribed for Child #9 could not be located during the inspection. Staff #1 and Staff #2 confirmed Child #9 attends the program some mornings.
