Inspector Notes
A renewal inspection was initiated on January 12, 2022 and concluded on January 18, 2022. There were twenty-one school age children present with four staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of three child records and three staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
1Evidence: The following was found during record review:
1. The record for child #1 (start date 9/7/2021) did not have father's home and employment information.
2. The record for child #2 (start date 9/20/2021) did not have father's home information, employment information for both parents, physician information, allergies, physical examination and immunization record.
3. The record for child #3 (start date 12/23/2021) did not have parents' employment phone numbers.
