Inspector Notes
An unannounced monitoring inspection was conducted September 11, 2023 from approximately 4:30 pm to 5:20 pm. Two staff were present supervising twenty school age children. Children were observed engaged in various table activities. Four child records and two staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
2Evidence: The most recent fire inspection available for review was dated 3/24/2022 which is more than a year old.
Based on review, facility did not ensure 2 of 4 child records have documentation of required information.
Evidence: The following was found during review of child records:
1. The record for child #1 (start date 8/21/2023) had documentation of a food allergy on the physical examination and there was not a written care plan including instructions from ap physician regarding the food to which the child is allergic and the steps to take in the event of a suspected or confirmed allergic reaction.
2. The record for child #3 (start date 8/21/2023) did not have parents? employment phone numbers.
3. The record for child #4 (start date 8/28/2023) did not have mother?s employment phone number.
