Inspector Notes

An unannounced renewal inspection was conducted from approximately 9:45 am to 10:40 am. At the time of entrance, 29 children were in direct care of 4 staff members. Ratios were in compliance. Staff and children were observed engaged in individual learning activities. The site was clean, organized and contained a varied supply of developmentally appropriate materials. Interactions between the children and staff were positive.

A total of 3 child records and 5 staff records were reviewed. The physical space, emergency supplies, evacuation drills, attendance records, required posted documents, and medications were also reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review and interview, it was determined that the center did not ensure that employees that had lived out of the state of VA within the last 5 years have in their files the result of an out-of-state Central Registry search request within 30 days of employment.

Evidence:
1) The record for Staff #3 (start date 07/14/2020) did not contain documentation of an out-of-state Central Registry search request. Staff #3 had declared in their sworn statement having lived out of the state of VA within the last 5 years before employment.
Plan of Correction: Will request the out-of-state background check ASAP.
