Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.
A monitoring inspection was initiated on August 10, 2021 and concluded on August 11, 2021. The staff in charge was was contacted by telephone and a virtual inspection was conducted. There were 33 children present with 4 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 3 child records and 3 staff records were reviewed. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.
Violations
4Evidence: These following hazardous chemicals were observed and have the statement ?Keep out of reach of children? and at least one of the other following statements ?warning?, ?flammable? and ?hazardous?.
1) In the Spanish room, in an unlocked cabinet there was one container of paint, one container of hand sanitizer and one spray bottle of a clear liquid, center staff #1 confirmed it was a disinfectant.
2) In the School Age room, in an unlocked closet there was nine containers of hand sanitizer and one spray disinfectant.
3) In the Pre K- room in the bathroom stored on a shelf and an unlocked cabinet there was three containers of hand sanitizer, six containers of disinfectant wipes, three bottles of spray disinfectant and one container of air freshener.
Evidence: In the Spanish room, in an unlocked cabinet there was one spray bottle with a clear liquid and it was not labeled with it's contents. The center staff #1 confirmed it was a disinfectant.
Evidence: On the playground there are three composite pieces of equipment that have inadequate resilient surfacing;
1)The small composite piece for children ages 2-5, and the large composite piece for School Age children have three slides each. The resilient surfacing (mulch) at the base of each slide chute and around the fall zones are immeasurable. There are also large patches of grass at the base of the slide chutes and around the fall zones.
2) The large climbing composite piece for School Age children, has inadequate resilient surfacing (mulch) around the fall zone as it is immeasurable.
A minimum of 6 inches of mulch is needed at the base of slide chutes and in the fall zone areas.
Evidence: The record for child #3 was determined incomplete as the following information was missing; proof of identity, immunization record and physical. Center staff confirmed missing information and first date of attendance for child #3 was 6-17-2021.
