Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A monitoring inspection was initiated on 5/12/2021 and concluded on 5/17/2021. The director was contacted by email to initiate the inspection. There were 11 children present and 3 staff. The inspector emailed the director a list of items required to complete the inspection. The Inspector reviewed 2 children?s records and 2 staff records submitted by the facility to ensure documentation was complete. Information gathered during the inspection determined non-compliance(s) with applicable standards or law and violations were documented on the violation notice issued to the facility. The Licensing Inspector has reviewed with the provider COVID-19 Essential Guidance for Child Care programs.

Based on record review it was determined the center did not ensure that the written record of children's injuries contained all of the required components.
Evidence:
The injury reports submitted for inspection dated 3/12, 4/1, 4/12, and 4/23 did not contain future action to prevent recurrence.
Plan of Correction: The center will ensure that all injury reports contain future action to prevent recurrence.
