Inspector Notes

An unannounced monitoring visit and facility tour was conducted on 9/13/23. At the time of inspectors' arrival there were 16 aged children in care with 5 staff members and 1 administrator. Children were observed interacting with staff, nap, and departure. A sample of 5 children's records and 4 staff records were reviewed. Injury reports were also reviewed. Director reported that medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.
Evidence:
1. The play kitchen equipment in both classrooms rocked when pulled on, presenting potential tip hazards.
2. The staff confirmed that the equipment was not anchored.
Plan of Correction: Staff will submit a maintenance request.

Based on record review and interview it was determined the center did not ensure that the record for each child contained all of the required information.
Evidence:
1. 5 out of 5 records did not contain the names of any previous or concurrent day care and/or schools attended by the child.
2. Staff confirmed that this information was not available during this inspection.
Plan of Correction: Staff will request this information from parents.
