Inspector Notes

An unannounced monitoring visit and facility tour was conducted on 09/13/2023. At the time of inspectors' arrival there were 16 preschool aged children in care with 5 staff members and 1 administrator. Children were observed interacting with staff, engaging in educational activities, and free play. A sample of 5 children's records and 4 staff records were reviewed. Injury reports and were also reviewed. Director reported that medications are not being administered at this time. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.

Based on observation it was determined the center did not ensure all areas and equipment of the center, inside and outside, were be maintained in a clean, safe and operable condition.
Evidence:
The wooden kitchen playsets in the classrooms rocked when pulled on presenting potential tipping hazards.
Plan of Correction: A work order will be submitted for equipment will be anchored.

Based on record review and interview it was determined the center did not ensure that all children's records contained all of the required information.
Evidence:
1. 5 of the 5 record reviewed did not contain documentation of previous or concurrent programs attended by the child.
2. The record for child #3 did not contain documentation of the father's employment.
3. Staff confirmed that this documentation was not available during this inspection.
Plan of Correction: A form will be revised or created to document this information.
