Inspector Notes

An unannounced, on-site monitoring inspection was completed on 10/15/2025, as a part of the licensure period. The on-site inspection began at 1:30 pm and ended at 2:40 pm. The inspector reviewed compliance in the areas listed above. There were 23 children present and 7 staff. The inspector reviewed 5 children?s records and 5 staff records. This inspection included a document review, tour of facility, interviews, observations and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-160-A
Each staff member shall submit documentation of a negative tuberculosis screening at the time of employment and prior to coming into contact with the children.

There was no TB screening for Staff #5, who was hired more than five months ago.
Plan of Correction: Staff will get TB test/screening.
Standard 8VAC20-780-160-C
A tuberculosis (TB) screening is required every two years from the last screening.

Staff #1 updated screening was due over two months ago.
Plan of Correction: Staff will get TB test/screening.
Standard 8VAC20-780-550-P
The center shall maintain a written record of children's serious and minor injuries in which entries are made the day of occurrence.

The injury report for child #6 dated 10/2/25 did not contain future action to prevent recurrence.
Plan of Correction: Staff will be retrained on completing injury reports.
Standard 8VAC20-780-60-A
Each center shall maintain and keep at the center a separate record for each child enrolled that contains all of the required information.

The records for child #2 and child #4 did not contain all of the required parental employment information.
Plan of Correction: Staff will ask parents for this information.
