Inspector Notes
An unannounced monitoring visit and facility tour was conducted on 2/15/2024. At the time of inspectors' arrival there were 67 school aged children in care with 3 staff members. Children were observed interacting with staff and hand washing. A sample of 5 children's records and 4 staff records were reviewed. Injury reports and medications were also reviewed. Areas of noncompliance are identified on the violation notice and were discussed with the director in an exit meeting at the conclusion of this inspection.
Violations
4Based on observation and interview it was determined the center did not ensure, when children are in ongoing mixed age groups, the staff-to-children ratio and group size applicable to the youngest child in the group was applied to the entire group.
Evidence:
1. At the time of inspector's arrival at 2:40 pm until 2:54 pm there were 3 staff with 67 children.
a. The youngest child in the group was 5 years old, requiring a ratio of 1 staff person per every 18 children.
b. The school dismisses to the center at 2:04 pm.
2. Staff #4 was taking children from the cafeteria to the gym for parent pick up through out this inspection, leaving the other 2-3 staff alone in the cafeteria with 60+ children.
3. Staff #1 and staff #2 confirmed the center was not in compliance with the required one staff person for every eighteen children during this inspection.
Evidence:
1. Staff #2 was observed telling children "stop talking" and "that means no whispering", without offering any redirection or constructive feedback.
2. Two children interviewed said "counselors are sometimes mean". One child reported staff #2 told him to "shut up, and wont let me ask questions".

Based on record review and interview it was determined the center did not ensure the written record of children's serious and minor injuries was completed on the day of the occurrence.
Evidence:
1. Child #4 was observed with a bandage on their hand, they reported they fell at the center on 2/12/24. There was no injury report for child #4 at the beginning of this inspection.
2. Staff #1 produced a injury report and stated that staff #2 completed the injury report today(2/13/24) for the injury that happened on 2/12/24. Staff #2 confirmed the injury occurred on 2/12/24.
Based on record review and interview it was determined the center did not ensure that a record for each child enrolled was maintained and kept at the center.
Evidence:
1. There was no record for child #3, at the center. Child #3 was in care during this inspection.
2. Staff #1 confirmed this record was not available during this inspection.
3. Staff #3 confirmed there was no electronic means to access the record at this center at the time of the inspection.
