During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated completed on October 8, 2024. The on-site inspection began at 3:40pm and ended at 5:25pm. The inspector reviewed compliance in the areas listed above. There were 52 children present and three staff. The inspector reviewed five children?s records on site and three staff records electronically on 10/11,10/15, and 10/22. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/31/24. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-160-C
At least every two years from the date of the initial screening or testing, or more frequently if recommended by a licensed physician or the local health department, staff members shall obtain and submit the results of a follow-up tuberculosis (TB) screening.
The records for Staff #2 and Staff #3 were missing documentation of a follow-up TB screening, which is required.
Plan of Correction: The program director will conduct an audit of staff files and ensure that TB screenings and/or tests are completed every two years.

Repeat Violation
When children are in ongoing mixed age groups, the staff-to-children ratio applicable to the youngest child in the group shall apply to the entire group (1:18 for school age eligible up to nine years).
At the time of the inspector?s arrival, Staff #1 was observed working alone with 23 children. Around 4:50pm, Staff #2 was observed working alone for approximately nine minutes with 23 children. For both instances, the required ratio for school age eligible up to nine years was not met.
Plan of Correction: The program director will ensure that staff are in ratio for the number of children. If a staff calls out, the proper chain of command will be notified and will engage the phone tree to find emergency coverage.

Repeat Violation
The center shall maintain a written record of children's minor injuries in which entries are made the day of occurrence. The record shall include all the required information.
1.12 injury reports reviewed were missing any future action to prevent recurrence of the injury.
2. Three reports reviewed were missing staff and parent signatures or two staff signatures.
Plan of Correction: The staff will be re-trained on how to properly complete an injury report for minor injuries, to ensure that they are completed in their entirety.
