During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site monitoring inspection was initiated and completed on 9/23/25. The on-site inspection began at 2:50pmand ended at 4:55pm. The inspector reviewed compliance in the areas listed above. There were 92 children present and five staff. The inspector reviewed five children?s records and five staff records electronically on 9/25/25. This inspection included document review, a tour of the facility, interviews, and observations. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 10/14/25. A POC submitted after this date will not appear on the public website.
Violations
6Documentation of tuberculosis (TB) screening shall be submitted at the time of employment and prior to coming into contact with children.
The TB documentation for Staff #1 (employed five months) was not submitted at the time of employment, which is required.
There was no documentation in the record of Staff #1 (employed for five months) to show that The Virginia Department of Education-sponsored orientation course was completed.
A disinfectant spray was found on the top shelf of a cabinet, out of reach of children. However, the cabinet was not locked.
The required ratio for children school age eligible up to 9 years is 1:18 (staff: children).
At the inspector's arrival, there were two staff members, supervising 58 five to nine year olds. The two staff present could supervise no more than 36 children. In addition, multiples times during the inspection, staff was observed leaving the above-mentioned group, leaving the other staff out ratio.
On the day of the inspection, children were observed eating a snack that was not listed on the menu as a substitute.
Staff records shall be kept for each staff person and contain all the required information.
The record for Staff # 2 (employed for four years) did not contain documentation to demonstrate that the individual possesses orientation required by the job position.
