Plan of Correction: Cleaning supplies are now being stored in a secured location. Staff received retraining on proper storage procedures and daily safety checks were implemented.

Plan of Correction: Staff were retrained on the call-out and tardiness procedures. The site supervisor will ensure that they contact the program director within 10 minutes prior to the program starting if staff have not arrived. In addition, a review of the staffing schedule has been conducted to ensure ratios are maintained at all times, including transitions.
