Inspector Notes
This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program
A monitoring inspection was initiated on 07/22/21 and concluded on 07/28/21. The provider was contacted by telephone and a virtual inspection was conducted. There were nine children present, ranging in ages from six months to 4 years, with two caregivers supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, special care and emergencies and nutrition. A total of two child records and two caregiver records were reviewed.
Information gathered during the inspection determined non-compliance with applicable
standards or law and violations were documented on the violation notice issued to the program.
Violations
3Evidence: 1) The record for Child #1, enrolled on 06/08/2021, did not contain a physical examination. 2) During interview, Caregiver #1 reported the physical examination for Child #1 has not been received to date.
Evidence: 1) The record for Caregiver #1 contained a TB screening test dated 06/07/2017. 2) During interview, Caregiver #1 reported a repeat TB screening has not been completed since 06/07/2017.
Evidence: 1) The record for Child #1, enrolled on 06/08/2021, did not contain documentation that the child has been adequately immunized. 2) During interview, Caregiver #1 reported the documentation was not requested before the child attended the family day home.
