Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety (SHSI) inspection was initiated on 07/22/2021 and concluded on 07/28/2021. There were nine children present with two caregivers supervising. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy requirements that are not covered in licensing standards were reviewed during this supplemental inspection.

Information gathered during the inspection determined non-compliance with applicable
standards or law and violations were documented on the violation notice issued to the program.

Based on a review of records and interview, the vendor did not ensure that one caregivers who works directly with children completed the department's health and safety update course by the required deadline.

Evidence: 1) The record for Caregiver #2, hired on 06/26/2019, does not contain a certificate of completion of the Annual Health and Safety Update Training. The training should have been completed within 1 year.

3) During interview, Caregiver #1 acknowledged Caregiver #2 did not complete the required Annual Health and Safety Update Training by the required deadline.
Plan of Correction: The caregiver is in the process of completing the training.
