Inspector Notes

An unannounced monitoring inspection was conducted on January 4, 2023. There were 74 children present with 15 staff supervising. The physical plant, emergency preparedness plans, menu's cots, medications and authorizations, training logs, attendance logs, emergency drill logs, children's records and staff records were reviewed. Areas of non-compliance were cited on the Violation Notice. The children were observed handwashing, singing, reading stories and participating in circle time activities. Standard 8VAC20-780-160-A
Based on a review of staff records, the center failed to ensure that tuberculosis test or screening documentation shall have bee completed within the last 30 calendar days of the date of employment.

Evidence:
Staff #5's date of employment was 07/29/2022, the date of the TB test was 08/06/2021.
Plan of Correction: Staff member will submit to a TB screening and provide results to the center.
Standard 8VAC20-780-510-E
Based on a review of medication, the center failed to ensure that it's procedures for administering medication shall include methods to prevent use of outdated medication.

Evidence:
Child #7's Epinephrine Injector has expired 12/2022
Plan of Correction: Called the parent and asked her to bring in new prescription.

We will review our plan ensure expiration date checks on medications.
