An investigation was begun in response to an allegation being received in our office.
Based on multiple staff interviews, record review and review of video documentation, the preponderance of evidence does not support the allegation. The complaint is not valid. Violations cited were found during the course of the investigation, and not part of the original complaint.
Children's records, staff records, physical plant. medications and injury reports were reviewed as part of the investigation.

Based on a review of video documentation, one child was not kept in sight and sound supervision in the Preschool 2 classroom.
Evidence:
On the video recording (at 17:32:18) Staff A walks to the classroom door to speak with a pick up person for a child, leaving her back to the classroom. A child at a table on the other side of the room, climbs onto the table, squats down, stands up, jumps, stomps his foot and waives his arms. The child sits down and climbs down off of the table before the staff person turn around to see him (17:32:58).
Evidence:

Plan of Correction: Staff will be reminded to face the classroom while speaking with people picking up children from the class.

Based on a review of video documentation, a staff person does not engage in interactions with the preschool children in the classroom.
Evidence:
Staff B is observed on the video recording (17:16:16) sweeping the classroom and spraying the tables and diaper changing pad with sanitizing solution. The only interactions with children are instructions to "clean up" for 8 minutes until another staff person arrives (17:24:20)

Based on a review of a video recording, a staff person and a child did not wash their hands after contact with body fluids.

Staff B was observed wiping a child nose on the video recording. Neither the child nor the staff person washed their hands after the contact with body fluids.

Plan of Correction: The staff person was immediately reminded to wash their hands after wiping a child's nose, or to wear gloves and wash their hands, and to encourage the child to wipe their own nose then go wash their hands.
