Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, necessary due to a state of emergency health pandemic declared by the Governor of Virginia.

A complaint inspection was initiated and concluded on 3/18/2021. A complaint was received by the department regarding allegations in the areas of background checks and staff records. The director was contacted by telephone to conduct the investigation. The inspector emailed the director a list of documentation required to complete the investigation.

The evidence gathered during the investigation supported the allegation of non-compliance with standards or law and a violation was issued.

Based on record review, the child day center did not obtain a copy of the results of a sex offender registry check from any state in which the individual has resided in the preceding five years prior to the 1st day of employment.
Evidence:
1. Staff A (hired on 1/04/2021) resided in Ohio in the last 5 years and the center did not obtain the sex offender registry check until 1/11/2021.
Plan of Correction: All out of state background checks will be completed prior the employee's first day.
