Inspector Notes
An unannounced monitoring inspection was conducted on 10/17/2023 from 9:40am to 1:22pm. At the time of entrance, 110 children were in care with 19 staff members present. Children were observed playing with toys, playing on the floor, listening to the teacher read a book, singing with music, playing with balls in the Fitness classroom, playing in centers, creating a ?my own library card? with glue, scissors and markers, playing with table toys, washing hands, making an ?N? with play dough, getting ready to go outside and confirming their presence during a head count with the teacher, playing outside, lining up to use the restroom, participating in circle time (what is over, under, above, next to and on top). Infants were being held, eating at the table, playing on the floor, washing hands and sleeping. A selection of (5) staff and (5) children records, medications, the physical space, evacuation drills, allergy action plans and attendance records were reviewed. The site was clean and organized and contained an abundant supply of developmentally appropriate materials. Bulletin boards and several other areas of the programming space was decorated with artwork and projects completed by the children. Interactions between the children and staff were positive. Areas of non-compliance are identified in the violation notice.
Please complete the ?plan of correction? and ?date to be corrected? for each violation cited on the violation notice and return it to the department within 5 business days from receipt. Please specify how the deficient practice will be or has been corrected. Please do not use staff names; list staff by positions only.
Any plans of correction not received within 5 business days will result in the plans not appearing on the Violation Notice and public website.

6record information check or the Sex Offender registry check from any state in which the
individual had resided in the preceding five years prior to the first day of employment and did not send or obtain the child abuse and neglect registry.
Evidence:
1. Staff #4 (Date of hire 8/15/2023) had the Sex Offender registry check completed on 8/28/2023 and was not completed prior to hire.
2. Staff #3's file (Date of hire 8/30/2023) did not have record of sending the Search of the Child Abuse and Neglect to California within 30 days and did not have the criminal history record for California or Alabama and it was not completed prior to hire.
3. Staff #2's file (Date of hire 9/10/2023) did not have record of sending the criminal history record for South Carolina and was not completed prior to hire.

2. Preschool Director completed a Search of the Child Abuse and
Neglect to CA for Staff #3 via email. Our request was denied
as CA is a closed-record state. Please see attached
documentation. Preschool Director is working on criminal
history for CA and AL. Going forward, all Criminal History
Records will be completed prior to date of hire.
3. Staff #2's Criminal History record for SC was sent and received on
10/17/2023. Please see attached. Going forward, all Criminal History Records will be completed prior to date of hire by Preschool Director.
Evidence:
1. Staff #1(Date of hire 5/01/2023) completed the orientation training first aid and CPR on 6/08/2023.
2. Staff #2 (Date of hire 9/01/2023), Staff #3 (Date of hire 8/30/2023) and Staff #4 (Date of hire 8/15/2023) did not have documentation of completing orientation training in first aid and CPR.

Based on observation, areas of the center inside were not maintained in a
safe and operable condition.
Evidence:
1. In the Penguin classroom, the paint was chipping (small quarter size circle) to the right of the blue chairs.
2. In the Ducklings classroom, a mattress plastic cover had 3 rips/tears (2 were approximately a 1/2 inch long and 1 was approximately 3 inches long)
classroom. Will be painted this weekend or next.

Based on observation, hazardous substances such as cleaning materials were not kept in a
locked place.
Evidence:
In the Bear Cubs classroom bathroom that children use, a bottle of bleach water was inside the (High) cabinet on the 3rd shelf and was not kept locked.
10/19/2023 and will be installed immediately.. Staff reminded in Staff Meeting on
10/18/2023 that hazardous substances need to be locked. See attached
meeting agenda.
Based on observation, children's records were not treated confidentially.
Evidence:
1. In the Ponies classroom, the allergy list was inside a plastic slide cover attached to the outside of the cabinet and was not kept confidential.
