8VAC20-780 Staffing and Supervision.

Inspector Notes

A self reported investigation concerning supervision was initiated on 06/18/2024 from a report received by the Fairfax County Licensing Office on 06/13/2024. The investigation was conducted on 06/18/2024 between 11:07am to 12:01pm, and completed on 06/18/2024. A tour of the facility and staff interviews were conducted and 2 staff files were reviewed. Based on the preponderance of evidence gathered, violations were cited.

Standard 8VAC20-780-340-A
Based on observation of the center's video recordings and staff interviews, staff did not ensure care, protection and guidance while supervising children.

Evidence:
1. On 06/13/2024, at approximately 8:50 a.m. (according to the video time stamp), Staff #1 and Staff #2 did not notice as 3 out of 9 children opened the door to the fitness room and exited unsupervised to the outdoor playground.
2. Staff #1 and Staff #2 confirmed they were unaware that the 3 children had left the room unsupervised until Staff #3 found the children alone. Staff #1 brought the children back to the fitness room.
3. Based on video footage, Staff #3 found the 3 children on the enclosed playground at approximately 8:52 a.m. (video time stamp). The children were left alone and unsupervised on the enclosed playground for approximately 1.5 minutes.
Plan of Correction: As noted above, we will review our supervision policy at our next faculty meeting. All new teachers will continue to get training in supervision upon hire. Audible door alarms are being purchased and installed on all exterior doors.
Standard 8VAC20-780-340-F
Based on observation of the center's video recordings and staff interviews, the center did not ensure that all children under 10 years of age were always within actual sight and sound supervision of staff.

Evidence:
1. On 06/13/2024, at approximately 8:50am (video time stamp), 3 children (Aged: 2 years) opened the exit door in the room, left the fitness room unsupervised, and entered the enclosed outdoor playground. These 3 children were not within actual sight and sound supervision by Staff #1 and Staff #2 for approximately 1.5 minutes.
Plan of Correction: Staff #1 and Staff #2 were terminated on 6/14/24. A copy of Goddard?s supervision policy was emailed to all faculty on 6/14/24 and will be reviewed in depth at our July Faculty Meeting on 7/17/24. We will continue to review the licensing standards on supervision as a part of our tri-annual review with faculty at our January, May, and September Faculty Meetings.
