During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 4/1/2025 and completed on 4/1/2025, as a part of the licensure period. The on-site inspection began at 11:00am and ended at 1:30pm. The inspector reviewed compliance in the areas listed above. There were 121 children present and 24 staff. The inspector reviewed ten children?s records and ten staff records on-site. This inspection included document review and a tour of the facility.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 8VAC20-780-270-A
The center must ensure that areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition.

In the Bear Cubs (18 months- 2 years) classroom, the interior side of the door had chipped wood that was peeling, one inch in length, at a low level accessible to children. In the Ponies (2 to 3 years) classroom, a section of the wall border was missing at floor level, exposing chipped wood and drywall and the side panel of a shelf had peeling tape, both in areas accessible to children. In the Raccoons (18 months to 2 years) classroom, a mirror was secured with six pieces of peeling tape and an electrical outlet was covered with peeling masking tape, both in areas accessible to children.
Plan of Correction: Will remove all the peeling tape and place a work order to repair the mirror and the bear cubs door.

REPEAT VIOLATION:
Up-to-date information is required to be requested from the parent for each child's record at least annually.

The information in the record of child #4 had not been updated in over 9 months.
