Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review and interviews.

A monitoring inspection was initiated on 11/04/2021 and concluded on 11/12/2021. The director was contacted by telephone and a virtual inspection was conducted. There were 129 children present, ranging in ages from 2 years to 5 years, with 21 staff supervising. The inspector reviewed compliance in the areas of administration, physical plant, staffing and supervision, programming, medication, special care and emergencies and nutrition. A total of 5 child records and 5 staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program. Based on record review, the center did not ensure to obtain fingerprint-based criminal history check determination letter prior to first day of employment for one staff person.
Evidence:
1) On the date of inspection, the record for Staff #3 (start date 08/30/2021) did not contain a current fingerprint test result. The record for Staff #3 did contain a determination letter for a previous employer however, a new fingerprint-based criminal history check should have been completed for Staff #3 prior to employment.
Plan of Correction: Staff #3 has an appointment on 11/19/2021 for fingerprinting.
