Violations

Standard 8VAC20-780-270-A
Based on observation, areas of the center were not maintained in a safe condition.
Evidence:
1) The floor in front of the bathroom of Primary 109 was missing a piece making the floor an uneven area and posing a tripping hazard.
Plan of Correction: Quote for repair was received prior to licensing inspection. Repair is scheduled for next week. Whole floor is replaced after school year on June 13.
Standard 8VAC20-780-280-B
Based on observation, hazardous substances such as cleaning materials, were not kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1) Cleaning products were observed accessible to the children as follows:
a] Primary 109 - 2 cans of Lysol aerosol and 1 container of Lysol spray were observed on the counter next to the sink.
b] Primary 108 - Clorox wipes, Lysol spray and Lysol aerosol were observed on the floor of an unlocked closet.
Plan of Correction: Corrected day of inspection.
Due to our hourly protocol to sanitize due to COVID protocols these items are frequently used.
Standard 8VAC20-780-280-H
Based on observation, purses were stored in areas accessible to children.
Evidence:
1) A purse was observed on the floor of an unlocked closet in the Toddler 2 classroom.
Plan of Correction: Corrected.
Purses did not contain "cosmetics, medication or other harmful agents" as describe in licensing.
Standard 8VAC20-780-380-A
Based on observation, posted daily schedules were not posted as required.
Evidence:
1) On the date of inspection, the following rooms did not have a daily schedule posted: Toddlers 1, Toddlers 2, Primary 104, Primary 108 and Primary 109.
Plan of Correction: Corrected day of inspection.
Standard 8VAC20-780-40-M
Based on observation, information about children's allergies, sensitivities, and dietary restrictions was not dated or kept confidential.
Evidence:
1) Allergy lists in the following classrooms were posted in a way that the information was not covered or protected to keep it confidential: Toddler 1, Toddler 2, Primary 104, Primary 104 and Primary 109.
Plan of Correction: Corrected day of inspection by covering with blank piece of paper and adding date.
Due to our COVID protocols, no one other than classroom staff enter the building so fortunately we did not have a privacy issue.
Standard 8VAC20-780-60-A
Based on record review, children's records did not contain all required information.
Evidence:
1) The record for Child #1 and Child #4 did not contain the work phone number of each parent who has custody.
Plan of Correction: Corrected. "When applicable, work phone number and place of employment for each parent" in the licensing language appeared to allow parents to leave blank.
