During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated on 5/27/2025 and completed on 5/30/2025, as a part of the licensure period, The on-site inspection began at 7:18 and ended at 8:15am. The inspector reviewed compliance in the areas listed above. There were 10 children present and 3 staff. The inspector reviewed 5 children?s records and 5 staff emergency contact information were reviewed on-site. Additionally 5 staff records were reviewed digitally on 5/30/2025. This inspection included document review, tour of the facility, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 6/19/2025. A POC submitted after this date will not appear on the public website.

The license and any other documents required shall be posted in a conspicuous place on the licensed premises. The findings from the most recent inspection, dated 12/16/2024, were not posted in a conspicuous place on the premises.

compliance with posting requirements.
Standard 8VAC20-780-510-G
Medication shall be labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given. There was an Epi Pen in the medication lock box that does not have a prescription label or any identifying information as to the owner, dosage amount or times to be administered. Additionally, there are two over-the-counter medications that were not labeled with the child?s name.
Plan of Correction: The center will remove all
unlabeled medications from the
medication lock box, ensure that
every medication is properly
labeled with the child?s name,
medication name, dosage, and
administration times, and retrain
staff on medication labeling
requirements to prevent
recurrence.
Standard 8VAC20-780-510-P
When an authorization for medication expires, the parent shall be notified that the medication needs to be picked up within 14 days or the parent must renew the authorization. Medications that are not picked up by the parent within 14 days will be disposed of by the center by either dissolving the medication down the sink or flushing it down the toilet. There is an Albuterol canister in the medication lock box that expired 4/2025.
