Inspector Notes

An unannounced renewal inspection was conducted on 04/20/2023. There were 12 non-resident children and 1 resident child, ages 8 months- 4 years, totaling 32 points, and 3 caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, environment and equipment, preventing the spread of disease, emergencies and nutrition. A total of 5 children?s records, 4 caregiver records, 1 household member record, and 1 medication were reviewed. The children were observed in learning activities, free choice play and during lunch. The inspector arrived for the inspection at 10:25 am and departed at 12:15 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Based on record review, the provider failed to ensure that the documentation of the negative tuberculosis screening/test was completed within the last 30 days.

Evidence: The record for Caregiver 3 contained a documented hire date of 03/07/2023. The date on the tuberculosis screening/test was 01/16/2023.
Plan of Correction: Will always make sure that the tuberculosis test is not older than 30 days at time of hire.
