Inspector Notes

An unannounced monitoring inspection was conducted on 10/02/2023. There were 12 non-resident children and 1 resident child, ages 3 months-4 years, totaling 34 points, and 2 caregivers present during the inspection. The inspector reviewed compliance in the areas of administration, personnel, household members, caregiver training, physical environment and equipment, care of children, preventing the spread of disease, emergencies, nutrition, and background checks. A total of 5 children?s records and 4 caregiver records were reviewed. The children were observed in organized group activities, in free choice play and having lunch. The inspector arrived for the inspection at 10:50 am and departed at 12:30 pm.

Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the provider.

Standard 22.1-289.035-A
Based on record review, the provider failed to ensure that all caregivers shall undergo a background check in accordance with the Code of Virginia prior to employment and every five years thereafter.

Evidence: The most recent criminal history record check for Caregiver 3 was dated 08/06/2018; there was not a follow-up criminal history record check obtained by 08/06/2023.
Plan of Correction: An appointment will be made to get the criminal record check completed.
Standard 8VAC20-800-570-A
Based on observations and record review, the provider failed to ensure that a caregiver does not exceed 16 points by using the required point system to determine if additional caregivers are needed.

Evidence: There were 12 non-resident children and 1 resident child, totaling 34 points, and 2 caregivers present during the inspection; the allowed 32 points was exceeded.
Plan of Correction: Adjustments will be made to children's daily attendance through the month of October to ensure that points are maintained.
