Inspector Notes

An unannounced on-site monitoring inspection was conducted and completed on 10/15/2025. The on-site inspection began at 9:45 am and ended at 11:05 am. The inspector reviewed compliance in the areas listed above. There were 10 children present and 2 caregivers; the point total was 23. The inspector reviewed 5 children?s records, 5 caregiver records, and 1 household member record.

The inspection included document review, tour of the home, interviews and observations.

Information gathered during the inspection determined non-compliance with applicable standards or law and are documented on the violation notice issued to the program.

The center must request a search of the central registry prior to the employee?s first day of employment.

Caregiver 5, who has been employed for more than one month, did not have a central registry search requested.
Plan of Correction: Will email the Office of Background Investigations to follow-up on how to successfully submit the request.
