During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
The violation notice was amended on 3/25/2026.
An unannounced inspection was initiated on 2/24/26 and completed on 3/25/26 in response to a complaint received by the licensing office on 1/13/2026 relating to allegations relating to supervision. The inspector reviewed 1 child record and 1 staff record on site. This inspection included tour of the facility, document review, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law, and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 4/2/2026. A POC submitted after this date will not appear on the public website.
Violations
1On January 6, 2026, Child #1 (age 6) reportedly informed staff they were not feeling well and did not appear to be acting like their normal self. Due to child #1 making noises in her sleep at rest time, staff #1 took the child?s temperature. Despite the child reportedly having a normal temperature, staff informed the child?s authorized pick-up person when they arrived to pick the child up that they were making noises in their sleep and ?acting funny.? Staff did not immediately contact any parent or emergency contact of the child?s abnormal behavior. Immediately following pick-up, the child received emergency medical care related to their breathing and was hospitalized as a result.
