Inspector Notes
An unannounced investigation was initiated and completed 10/25/2024 in response to a complaint received by the licensing office on 10/21/2024 relating to allegations of Special feeding needs, Daily activities for infants, Hand washing and toileting procedures, Ratio and Care and Protection. The inspector reviewed 3 children?s records and 4 staff records onsite.
This inspection included document review, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegation(s); therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/20/2024. A POC submitted after this date will not appear on the public website.
Standard 8VAC20-780-340-A
When staff are supervising children, they shall always ensure their care, protection, and guidance.
infants were observed sleeping with a bib and pacifier attached to a ribbon presenting a strangulation hazard.
Plan of Correction: On 10/25/24, staff immediately removed all bibs and pacifiers on sleeping children. Practiced safe sleep Infant care training CUR105 was given to the teacher to complete by 11/22/2024.
Standard 8VAC20-780-370-1
When an infant is able to easily turn over from the back to the belly position and he is placed in his crib, he shall still be put on his back but allowed to adopt whatever position he prefers.
Staff #1 was observed placing Child #2 (9 months) in their crib on their belly.
Plan of Correction: On 10/25/24, the school's director printed out and reviewed the licensing regulation with staff members. Each party demonstrated safe sleep and placing the child in the crib. Infant/toddler CCEI 900 care was given to the teacher to complete by
11/22/2024.
Standard 8VAC20-780-420-E-1
For each infant, the center shall post a daily record which can be easily accessed by both the parent and the staff working with the child.
On 10/1/2024 and 10/2/2024, the center did not post a daily record which could be accessed by both parents and staff working with Child #3. On both days the center failed to document the correct amount of food consumed by the child that day nor was time documented for how much time the child spent on their stomach.
Plan of Correction: On 10/25/24, training on time management was implemented. The director will ensure staff members are inserting the accurate feeding times as well as tummy time. A daily tummy time chart is listed for a daily visual for the parents and staff. Infant care training CUR105 was given to the teacher to complete by 11/22/2024.
Standard 8VAC20-780-500-A
Center shall ensure to follow handwashing procedures for both staff and children.
Staff #1 did not wash hands before or after changing a child?s diaper. Child?s hands
were also not washed after the diaper change.
Plan of Correction: On 10/25/24, the school's director had staff #1 perform the proper handwashing with assistance at first, then observed to make sure the staff member will properly do the steps going forward. Reviewed regulations. The training" Hand hygiene in early care and education" was completed by staff on 11/15/2024.
Standard 8VAC20-780-570-A
When a child is placed in an infant seat or highchair, the protective belt shall be fastened securely.
Child #1 (9 months) was not securely fastened at the feeding table. The straps that are supposed to be connected to the seat were not present.
Plan of Correction: The safety strap was being washed. On 10/25/24, the school director immediately provided the staff with a replacement strap for the chair on site 10/25/2024. Infant care training CUR105 was given to the teacher to complete by 11/22/2024.
Standard 8VAC20-780-80-A
For each group of children, the center shall maintain a written record of daily attendance that documents the arrival and departure of each child in care as it occurs.
In the Infants classroom there were 5 children signed in electronically and only 4 children were present in the classroom.
Plan of Correction: This was corrected on site on 10/25/2024. Child was properly signed into classroom on 10/25/2024. Safety infant/ toddler care CCEl900 was assigned to staff to be completed by 11/22/2024.
Moving forward, staff members will ensure that all children are signed out on the rollcall sheet while in transition to another room.
