During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced, on-site monitoring inspection was initiated and completed on 8/11/2025. The on-site inspection began at 9:55am and ended at 1:10pm. The inspector reviewed compliance in the areas listed above. There were 95 children present and 13 staff. The inspector reviewed 8 children?s records and 5 staff records on-site.

This inspection included document review, tour of the facility and interviews.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 8/25/2025. A POC submitted after this date will not appear on the public website.

Standard 8VAC20-780-130-E
The center shall obtain documentation of additional immunizations once every six
months for children under the age of two years.

1. The immunization records for Child C (Age 3) indicates the child?s most recent immunizations were administered over 1 year 9 months.
Plan of Correction: On 8111 /2025 the parents of Child C were made aware that they needed updated immunizations records. 8/13/2025 They provided the updated documentation.
Standard 8VAC20-780-150-B
Immunization records are required to be signed and dated by a physician, his designee, or an official of the local health department.

The immunization record for Child B, enrolled for 5 months, was not signed or dated.
Plan of Correction: On 8/11/2025, the parent was notified that updated immunization forms were needed. She scheduled an appointment on 8/12/2025 for 8/21/2025 for updated immunizations.
Standard 8VAC20-780-280-B
Hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.

1. In the Infant A classroom, there was a bottle of sanitizing spray on the counter, and disinfecting spray unlocked in an upper cabinet.
2. In the Infant B classroom, there was a spray bottle of sanitizing and disinfecting spray in an unlocked upper cabinet.
3. In the Toddler A classroom, there was a spray bottle of disinfectant unlocked in a upper cabinet.
Plan of Correction: On 8/11/2025 in the Infant A classroom the spray bottle was moved to a locked cabinet onsite. The correct lock was placed on the cabinet with the disinfectant spray in it.
On 8/11/2025 The lock was clasped together on site.
On 8/11/2025 the correct lock was placed on the Toddler A cabinet on site.
Standard 8VAC20-780-40-M
The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan. This list shall be dated and kept confidential in each room or area where children are present.

1. The allergy list in the Preschool 2A classroom was not kept confidential, it is posted with no cover.
2. Child A who had started care on day of inspection, was not on the current list in the school-age classroom of children with allergies.
Plan of Correction: On 8/1112025 The allergy list in Preschool 2A was moved on site to the inside of the cabinet under the cover sheet. On 8/1112025 Child A was added to all allergy list and posted in the school age classroom.
