Inspector Notes
A joint inspection was initiated on 1/19/2023 with the local department of social services to investigate a self-reported incident regarding supervision of children and care, protection and guidance. Interviews were conducted and documentation was reviewed. Additional interviews were conducted on 1/30/23, 2/9/23, and 2/13/2023. Based on evidence obtained, a violation notice was issued to the facility. An exit interview was conducted on 3/13/2023.

Violations
5Evidence:
1) On 1/12/2023, from approximately 5:20 - 6:33pm staff were unaware that child #1 (age three) was sleeping on the floor in the back of the preschool II B classroom.
2) Three staff had responsibility for the supervision of child #1, and the three staff did not observe child #1, check the attendance roster, count the children, or take attendance during this time.
3) Staff #2 entered the preschool II B classroom at approximately 5:20pm, to relieve Staff #1, and proceeded to sit down and color with the children at the table. Staff #2 did not check the attendance roster, or walk through the classroom, therefore was unaware that child #1 was asleep in the back of the classroom.
4) Management #3 was not aware that child #1 was asleep in the back of the preschool II B classroom when they "peeked" into the classroom and told staff #2 to transition the five children to the school age classroom.
5) Staff #4 was not aware that child #1 was supposed to be in their classroom when five children transitioned from the preschool II B classroom to the school age classroom at approximately 5:30pm. Staff #4 did not take attendance, check the attendance roster or count the children entering their classroom.
6) Staff were unaware that child #1 was asleep in the preschool II B classroom until staff #5, accompanied by a set of parents, discovered the child at approximately 6:33pm.
Evidence:
1) On January 12, 2023, at approximately 5:20pm, Staff #2 entered the preschool IIB classroom to relieve Staff #1, and stated that they never saw child #1, (age three).
2) Staff #2 did not know that child #1 was asleep in the back of the classroom, therefore did not have sight supervision from approximately 5:20 ? 5:30pm
3) At approximately 5:30pm, Staff #2 moved five children from the preschool II B classroom to the school age classroom. Staff #2 stated that they turned off the lights and closed the preschool II B classroom door. Child #1 was left alone in the classroom.
4) Staff #4 entered the preschool IIB classroom at approximately 6:33 pm on January 12, 2023, with a set of parents, and discovered child #1 asleep on the floor, behind a book shelf in the back of the preschool II B classroom. Staff #4 stated that they picked up the child, and handed the child to management #3. Child #1 had been alone in the classroom, out of sight and sound supervision, from approximately 5:30 - 6:33pm.
Evidence: On January 12, 2023 at approximately 5:20pm, there was one staff member alone with 17 children in the school age classroom. The youngest child in the room (Date of birth: 9/3/18) was four years old. At approximately 5:30pm, five additional children were transitioned to the school age classroom, leaving 20 children with one staff member (two children had left). The youngest child in the room (Date of birth 8/5/19) was three years old.
Evidence: The center's policy on classroom attendance states, "When a class is moved from their room for any reason, attendance is taken by matching faces to names on the attendance sheet. Before leaving any location, employees are to check the area, including bathrooms, behind furniture, doors and learning equipment for children. Employees document this procedure on the Classroom Movement Log, and then have it verified by management, before children leave the room." On the incident date of January 12, 2023, staff #2 and management #3 did not follow the center's attendance policy, which resulted in leaving a three year old child alone in a classroom for approximately one hour.
