Inspector Notes

An unannounced monitoring inspection was conducted on 11/27/2023 12:40 pm to 1:45 pm and on 11/30/2023 from 2:00pm to 4:20pm. At the time of entrance, 67 children, ages 6 months to 4 years old, were in care with 10 staff members present. The sample size consisted of 6 children?s records and 7 new staff records. Children were observed sleeping during rest period, engaged in individual and small group play, having an afternoon snack during snack time. Medication was reviewed with regard to storage, expiration dates and written authorization and required documentation. Information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program

Based on a review of records and interview, the licensee did not always maintain and keep at the center a separate record for each child enrolled which contained all of the required information:
Evidence:
Six children's records were reviewed.
The record for child #6 did not contain the name, telephone number and address for 2 people to be contacted in an emergency if the parents cannot be reached.
Child #6's record listed the child's mother and father as the emergency contacts.
The program director acknowledged that emergency contacts were not listed in the record for child #6.
Plan of Correction: Child #6's parent will be asked to provide contact information for two individuals other than child #6's mother and father in case they cannot be reached or available in an emergency. This information will be documented in child #6's record by the end of the week.
