Inspector Notes

In response to a complaint received by the licensing office on 12-18-2024 relating to allegations of Staff Qualifications and Training and Staffing and Supervision. This inspection included document review, tour of the facility, interviews and observations. The inspector reviewed 10 staff records on-site. This inspection included document review, tour of the facility, interviews and observations.
The preponderance of evidence gathered during the investigation supports the allegation; therefore, the complaint is determined to be valid. Information gathered during the inspection determined non-compliance with applicable standards or law and the violations are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 1-9-2025. A POC submitted after this date will not appear on the public website.

Each grouping of children shall have at least one staff regularly present who meets lead teacher qualifications. Staff #1, who does not meet lead teacher qualifications, was the designated teacher working with a group of infants on December 18, 2024 and on December 31, 2024.
Plan of Correction: Staff member had a 10-hour preservice training, as well as 19 hours of in-house training. We were unaware that the 10-hour training was outside of the 6-month qualifying window. As of 01/01/2025, staff member has received the remaining hours needed to qualify as a Lead Teacher. Moving forward, management will ensure all staff receive the required 24-hour training within a 6-month time frame prior to being a LT.
