During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced, on-site renewal inspection was initiated and completed on 4-29-2025. The onsite inspection began at 1:45pm and ended at 5:10pm. The inspector reviewed compliance in the areas listed above. There were 74 children present and 9 staff. The inspector reviewed 7 children?s records and 7 staff records on-site. This inspection included document review, tour of the facility, interviews, observations, and measurements. Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program. Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five business days from today, which will be the close of business on 5/12/2025. A POC submitted after this date will not appear on the public website.
Violations
12Areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment. On the ?Older? children?s playground there were areas of rust, chipping and peeling paint on the metal playground piece. The poured in-place surfacing was coming apart from the concrete sidewalk, there were areas of the broken poured in-place surfacing that were filled in with concrete and the concrete was broken/ cracked and areas of raised surfing /warping and cracked surfacing. These areas pose a hazard.
Cribs, cots and rest mats shall be identified for use by a specific child. Child #8 was observed sleeping in a crib labeled for another child.
The center shall implement a monthly practice evacuation drill. There was not documentation of drills conducted in January, February and March 2025.
1.The records for Child #3 and Child #5 did not include employment phone number for one parent and the required annual record update, both records were overdue an update by approximately 16 months.
2. The records for Child #4 and #7 did not include an employment phone number and location for one parent.
3. The record for Child #2 did not include the previous school (childcare) attended
All required documentation shall be maintained for staff records. Staff #2 and Staff #4 did not have documentation kept at the center regarding any health problems that may interfere with fulfilling the job responsibilities.
A written agreement between the parent and the center shall be in each child's record by the first day of the child's attendance. The agreements listed below shall be signed by the parent(s).
1. An authorization for emergency medical care should an emergency occur when the parent cannot be located immediately unless the parent states in writing an objection to the provision of such care on religious or other grounds.
2. A statement that the center will notify the parent when the child becomes ill and that the parent will arrange to have the child picked up as soon as possible if so requested by the center.
3. A statement that the parent will inform the center within 24 hours or the next business day after his child or any member of the immediate household has developed any reportable communicable disease, as defined by the State Board of Health, except for life threatening diseases which must be reported immediately.
Child #1 (attending for more than 2 ? years) and Child #6 (attending for more than 3 years) did not have agreements 1, 2 and 3 signed in their record, Child #3 (attending for more than 2 years) did not have agreement 2 and 3 signed in their record and Child #5 (attending for more than 2 years) did not have agreement 2 signed in their record.
