An unannounced monitoring inspection was initiated on 7-22-2025 and completed on 8-12-2025, in response to a self-report received by the licensing office on 7-21-2025 relating to area or part of standard, Staffing and Supervision and Programs. The inspector reviewed 1 staff record on-site and electronically on 7-22-2025 and electronically from 7-22-2025 up to 8-12-2025. This inspection included document review, tour of the facility, interviews and observations.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Behavioral guidance shall be constructive in nature, age and stage appropriate, and shall be intended to redirect children to appropriate behavior and resolve conflicts.
On July 18, 20025, two children, age 4 (Child #1 and Child #2) in Classroom A were observed pushing each other back and forth 3 times and then on the fourth push Child #1 fell backwards into a piece of children?s furniture. Staff #1 did not intervene, and Child #1 was observed crying.
Plan of Correction: The teacher will receive a written disciplinary action for Behavioral Guidance and will be put on a Teacher Performance Improvement Plan.
