Repeat Violation
Areas and equipment of the center, inside and outside, are maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment.
1.On the Preschool playground there were areas of rust on the metal bars on the playground composite piece.
2.On the schoolage playground the poured in-place surfacing was coming apart from the concrete sidewalk, there were areas of the broken poured in-place surfacing that were filled in with concrete and the concrete was broken/ cracked and areas of raised surfing /warping and cracked surfacing.
3. In room E, the lid on the water tank on a children?s toilet was not secure on the top of the tank.
4.In room D, near a children?s toilet there is a section of floor covering, approximately 2 feet long that is cracked, broken and has sharp edges.
Plan of Correction: The facilities manager is working on a plan for the rust and the surfacing on the playground or will contract it out.
The water tank lid was secured during the visit.
The flooring in the twos bathroom will be repaired or replaced.

The center shall maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions documented in the allergy plan. Child #8 has a dietary restriction and is served alternate milk and it was not listed.

Children's hands shall be washed with soap and running water after toileting and any contact with blood, feces, or urine. In room A diapering was observed and the Staff #7 did not wash Child #7?s hands after diapering

Repeat Violation
Disposable diapers shall be disposed in a leakproof or plastic-lined storage system that is either foot-operated or used in such a way that neither the staff member's hand nor the soiled diaper touches an exterior surface of the storage system during disposal. The diapering surface shall be cleaned with soap and at least room temperature water and sanitized after each use.
1.In rooms A and B there were not a trashcans/disposal system that had a lid as required for soiled disposable diapers. Soiled diapers were observed in a trash can that did not have lid as required.
2. Diapering was observed in Room A, and the Staff #7 did not wash the diaper pad after changing the diaper. Soap and water were not available for washing the diaper pad.
Plan of Correction: Teachers were retrained on how and where to dispose diapers. Teachers were also retrained on the diapering process.
