Inspector Notes

This inspection was conducted by licensing staff using an alternate remote protocol, including telephone contacts, documents review, interviews and a virtual tour of the program.

A subsidy health and safety inspection was initiated on 09/13/2021 and concluded on 09/16/2021. There were 11 children present with 2 staff. A licensing inspection was also conducted on this date. Both inspections combined cover subsidy health and safety regulations. Only subsidy regulations that are not covered in licensing standards are reviewed during this supplemental inspection.

The information gathered during the inspection determined non-compliance with applicable standards or law and violations were documented on the violation notice issued to the program.

Standard 22VAC40-665-500-F
Based on record review the vendor failed to ensure that the written list of all children's food sensitivities, and dietary restrictions was current.

1. The enrollment record for child 1 indicated a sensitivity to peanuts however, this food sensitivity was not listed on the written food allergy/sensitivity list submitted to the inspector.
Plan of Correction: The food allergy list was updated to include this child's food sensitivity.
Standard 22VAC40-665-540-A
Based on record review, the vendor failed to ensure that the required TB screening shall have been completed within the last 30 calendar days of the date of employment.

Evidence:
1. The TB screening for staff 1 was completed on 11/09/2020 which is more than 30 days prior to her hire date of 05/03/2021.
Plan of Correction: All new staff will have a TB screening completed within 30 days of employment.
