During the inspection, the inspector reviewed the areas listed above to include standards found out of compliance during the previous inspection. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.

Inspector Notes

An unannounced , on-site monitoring inspection was initiated on 02/04/2025 and completed on 02/04/2025. The on-site inspection began at 2:55 pm and ended at 5:16 pm. The inspector reviewed compliance in the areas listed above. There were 41 children present and 6 staff. The inspector reviewed 5 children's records on-site and 3 staff records on site.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the deficient practice will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 02/13/2025. A POC submitted after this date will not appear on the public website.

Repeat Violation
The center shall obtain a copy of the results of a criminal history record information check from any state in which the individual has resided in the preceding five years.

There was no criminal record child on file for staff 2 who has resided in a state outside of Virginia within the past 5 years.
Plan of Correction: A written request has been submitted and documented to obtain the criminal history for staff member who has lived out of state.
Standard 8VAC20-780-240-A
. The Virginia Department of Education-sponsored orientation course shall be completed within 90 calendar days of employment.

Staff 1 has not completed the Virginia Department of Education-sponsored orientation course. Staff 1 has been employed for more than 90 days.
Plan of Correction: Staff member has completed her training.

Parents shall be provided at least semiannually in writing information on their child's development, behavior, adjustment, and needs. Such sharing of information shall be documented.

There was not written documentation of an annual update of the information in the enrollment record of child 1. Child 1 has been enrolled for more than 4 years.
Plan of Correction: The electronic annual updates will be printed and placed in each child's file.

A first aid kit shall be wherever children are in care.

There was no first aid kit on the van used to pick up children from 4 local public schools. The van returned to the facility at approximately 3:05 pm with children on board.
Plan of Correction: The first aid kit had been moved to the trunk of the van. The kit will remain in the front of the van at all times.

Repeat Violation

Shelter-in-place procedures or maps shall be posted in a location conspicuous to staff and children on each floor of each building.

Shelter-in-place procedures were not included on the posted evacuation map for the facility.
Plan of Correction: The Shelter In Place designation has been added to the existing Evacuation Map that is posted by each exit.
Standard 8VAC20-780-580-C
The center shall ensure that during transportation of children the following information is in transportation vehicles:
*A list of the names of the children being transported.

A list of the names of the children being transported was not on the facility van when the vehicle arrived from local public schools at approximately 3:05 pm.
Plan of Correction: An updated checklist has been copied and placed on the van to record children being transported.

Staff records shall contain all of the required elements.

The employment file for staff 1 and staff 2 lack written documentation of completion of orientation training.
Plan of Correction: Orientation training documentation has been placed in the files of the mentioned staff members.
