Inspector Notes

The Licensing Inspector initiated and completed an unannounced renewal inspection on 3/15/2023. The inspector was on site from 8:00am until 11:40am.

There were 63 children under the care of 11 staff. The ages of the children ranged from five months to five years old. The children were observed during indoor free play, breakfast, group time and lunch. Six children?s record and four staff records were reviewed.

Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Standard 22.1-289.035-B-2
Based on record review, the center failed to ensure a Criminal History check was completed prior to first day of employment.

Evidence:

Staff #2 had a documented employment date of 5/4/2018. Then Criminal History was dated for 6/12/2018.

Staff #3 had a documented employment date of 4/9/202. The Criminal History was dated for 4/12/2021.
Plan of Correction: Policy has been put in place that Criminal History will be completed before first day of employment.
Standard 8VAC20-770-60-B
Based on record review, the center failed to ensure Sworn Statements were completed prior to first day of employment.

Evidence:

Staff #1 had a documented employment date of 4/23/2018. The sworn statement was dated 4/18/2019.

Staff #2 had a documented employment date of 5/4/2018 The sworn statement was dated for 6/18/2018.
Plan of Correction: Policy has been put in place to ensure that sworn statement is completed prior to first day of employment.

Based on record review, the center failed to ensure Central Registry results by the end of thirty days of employment.

Evidence:

Staff #2 had a documented employment date of 5/4/2018. The Central Registry results letter was dated for 10/20/2022.
Plan of Correction: Policy has been put in place to that Central Registry will be completed first day or orientation.
Standard 8VAC20-780-280-B
Based on observation, the center failed to ensure hazardous substances such as cleaning materials, shall be kept in a safe locking method that prevents access by children.

Evidence:

Classroom #4 had cleaning chemicals in the cabinet under the sink. There was no lock on the cabinet, making it accessible to children.
Plan of Correction: Lock was fixed at time of inspection on 3/15.
Standard 8VAC20-780-440-B
Based on observation, the center failed to ensure cribs shall be identified for use by a specific child.

Evidence:

There were approximately nine cribs in the sleeping area of classroom #1. There was no label on the cribs to identify use of specific children.
Plan of Correction: Cribs were labeled at time of inspection on 3/15.

Based on record review, the center failed to ensure documentation of two or more references with date of contact.

Evidence:

Staff #4 had references on file during the inspection, but there was no date as to when these references were checked.
Plan of Correction: Human Resource will ensure that references have dates 3-15.
