Inspector Notes
The Licensing Inspector initiated and completed an unannounced renewal inspection on 3/15/2023. The inspector was on site from 8:00am until 11:40am.
There were 63 children under the care of 11 staff. The ages of the children ranged from five months to five years old. The children were observed during indoor free play, breakfast, group time and lunch. Six children?s record and four staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations
6Evidence:
Staff #2 had a documented employment date of 5/4/2018. Then Criminal History was dated for 6/12/2018.
Staff #3 had a documented employment date of 4/9/202. The Criminal History was dated for 4/12/2021.
Evidence:
Staff #1 had a documented employment date of 4/23/2018. The sworn statement was dated 4/18/2019.
Staff #2 had a documented employment date of 5/4/2018 The sworn statement was dated for 6/18/2018.
Evidence:
Staff #2 had a documented employment date of 5/4/2018. The Central Registry results letter was dated for 10/20/2022.
Evidence:
Classroom #4 had cleaning chemicals in the cabinet under the sink. There was no lock on the cabinet, making it accessible to children.
Evidence:
There were approximately nine cribs in the sleeping area of classroom #1. There was no label on the cribs to identify use of specific children.
Evidence:
Staff #4 had references on file during the inspection, but there was no date as to when these references were checked.
