Inspector Notes
An unannounced renewal inspection was initiated and completed on 8/17/2023. The inspector was on site from 11:10am until 2:30pm.
There were 80 children, ages 3 months to 5years old under the supervision of 16 staff. Children were observed during indoor free play, lunch time and nap. Five children's records, two medical records and five staff records were reviewed.
Information gathered during the inspection determined non-compliance with applicable standards or law, and violations are documented on the violation notice issued to the program.

Violations
6Evidence: Staff #5, hired on 6/27/2019 had no record of a Central Registry on file during inspection on 8/17/2023. Administration was unable to provide documentation when requested.
Evidence:
1.The record for Child 4 did not contain documentation that they received the immunizations required by the State Board of Health. Staff 6 verified that the first day of attendance for Child 1 was 08/16/2023.
Evidence: Staff #5 which was hired on 6/27/2019 had no record of a TB screening on file during the inspection on 8/17/2023.
Based on observation, the center failed to maintain areas and equipment of the center in a clean, safe operable condition.
Evidence:
The sandbox on the playground had very little sand inside. The mesh of the bottom was coming through causing a tripping hazard to children.
Evidence:
1.In the infant classroom, there were bottles in the fridge that had no label to show ownership or dates.
2.In the Infant 2 class, there were bottles in the fridge with no label to show ownership or dates.
Evidence:
1.Child #4, had an enrollment date of 8/1/2023 had no proof of ID on file during the inspection on 8/17/2023.
