Inspector Notes

Licensing Inspector Anderson conducted an unannounced complaint inspection in response to allegations received by the licensing office on 10/25/2023, in the areas of forbidden actions and staffing & supervision. The investigation of the complaint was initiated on November 30, 2023 and findings were reviewed on December 7, 2023.

As the information gathered during the investigation does not support the allegations, the complaint is determined to be ?NOT VALID.?

Standard 8VAC20-780-160-C
Based on record review, the Center failed to ensure at least every two years from the date of the first initial screening or testing, staff members obtain and submit the results of a follow-up tuberculosis screening/testing.

EVIDENCE:
Staff #1 did not have an updated tuberculosis skin test/screening completed every 2 years. Most recent tuberculosis skin test for Staff #1 was completed on 10/01/2021.
Plan of Correction: Staff #1 has an appointment scheduled for her TB. Will have this completed no later than 12/14/2023

Based on record review, the Licensee failed to maintain children records to ensure all required information is documented.

EVIDENCE:
Record for Child #1 did not include the name address and telephone number of at least two designated people to call in an emergency if a parent cannot be reached. Record was missing the information for 1 emergency contact.
Plan of Correction: Waiting on a response from mom for the 2nd emergency contact. Will have this completed no later than 12/14/23
