During the inspection, the inspector reviewed the areas listed above. Unless otherwise noted as a violation within this inspection report, the provider was in compliance with the standards reviewed. If there were any serious injuries or fatalities related to a violation, the details will be included in the description of the violation.
Inspector Notes
An unannounced inspection was initiated on 10/4/2024 and completed on 10/29/2024 in response to a complaint received by the licensing office on 10/3/2024 relating to allegations of supervision. inspector reviewed 1 child?s records and 2 staff records on-site. Additional interviews were completed on 10/17/2024. This inspection included document review, tour of the facility, and interviews.
The preponderance of evidence gathered during the investigation does not support the allegation(s); therefore, the complaint is determined to be not valid. Though not a part of the original complaint, violations that were not reported were found related to children?s records and background checks are documented on the violation notice issued to the program.
Please complete the plan of correction (POC) and date to be corrected sections for each violation cited on the violation notice. Specify how the violation will be or has been corrected. Submit your POC within five (5) business days from today, which will be the close of business on 11/4/2024. A POC submitted after this date will not appear on the public website.
Violations
3The center is required to obtain background checks from any stat in which the individual has resided in the preceding five years.
Staff 2, who has been employed for more than two months, did not have documentation of requestions a central registry check from a state.
Staff 1 and 2 was observed working on the day of the inspection. Neither had documentation of the educational requirements for a program leader position.
Child 1, enrolled over three years, was missing proof of identity.
